Check Your Progress Forms

You can use these forms anytime to keep track of or check your progress. The fewer symptoms you are having or the less severe your symptoms, the more balanced your physiology is becoming. Take them every 6-8 weeks and keep them in a binder to track your progress (or challenges). You can bring them to your appointments.

Remember: even failures and “falling off the wagon” can be useful experiences, as they teach us that the things we were doing were actually helping!
